BROC and Humana have been unable to reach agreement on a new provider contract despite extensive discussions. Our hope remains that an agreement can still be reached before September 1 and that any disruption to patient care can be avoided.

Yes. Unless a new agreement is reached, BROC will become out-of-network for all Humana insurance products effective September 1, 2026. Patients are encouraged to contact Humana directly to better understand how this change may affect their individual coverage.

Depending on your specific Humana plan, you may still be able to continue receiving care from your BROC physician as an out-of-network patient. Out-of-network benefits, referrals, authorizations, and continuity-of-care requirements vary by plan.

Patients with surgeries, procedures, therapy visits, injections, imaging appointments, or office visits already scheduled should contact BROC as soon as possible. Our team will work with you to review your options and minimize disruptions to your care whenever possible.

If your surgery is scheduled prior to September 1, 2026, your care should generally continue under BROC's current in-network participation status, subject to your plan benefits and any required authorizations.

Please contact BROC and Humana as soon as possible. Depending on your individual circumstances, options may be available to continue care with your BROC physician.

BROC is committed to helping patients continue established treatment plans whenever possible. However, because out-of-network care often requires additional administrative approvals and payer requirements, appointment availability for Humana patients may be more limited than it is today.

Continuity of care refers to situations where a health plan may allow patients to continue treatment with their current physician for a limited period of time following a network change. Eligibility requirements vary by plan and patient circumstance. Patients should contact Humana directly for more information.

Depending on your Humana plan, a referral from your PCP and/or additional approvals from Humana may be required to continue receiving care from an out-of-network provider. Patients should contact both Humana and their PCP's office to understand what requirements apply to their specific situation.

Contact your PCP and let them know that you would like to continue receiving care from your current BROC physician. Ask whether a new referral, referral update, continuity-of-care request, or other documentation is needed to support ongoing treatment with an out-of-network orthopedic provider.

• Will I need a referral to continue seeing my BROC physician?
• Do I qualify for continuity-of-care consideration?
• Will I need prior authorization for future visits, therapy, imaging, injections, or surgery?
• What will my estimated out-of-pocket costs be if I continue receiving care at BROC?
• What in-network orthopedic providers are available in my area?

Patients with concerns regarding this network change are encouraged to contact Humana Member Services using the phone number on the back of their insurance card. Patients may also discuss the impact of this change with their Primary Care Physician.

Medicare beneficiaries may have opportunities during annual enrollment periods or special enrollment periods to review and make changes to their coverage. Patients should contact Medicare, Humana, or a licensed Medicare advisor to discuss their options.
